How about the fit for a 44" waist with a s&w m&p 9 shield?

I have a large waist but I carry a Sig p238. It is very comfortable. I would imagine it would work.

Will this fit a M&P Shield with built-in guard laser?

Yes! I carry my double stack Glock 30 45cal and my S&W full size 9mm with an under barrel laser/flashlight. However, the laser catches the bottom of the holster when pulling the gun out. A gun that has a smooth barrel without attachments will pull smoothly.
